2. Personal information consent form
I understand that the Orca Synchronized Swimming Club collects personal information about each of its registrants, including name, address, email, telephone number, sex, age, and date of birth. This information is used for the purposes of communications from the Orca Synchronized Swimming Club and in determining demographics and market trends. The information is also used by and disclosed to Sport Canada, Synchro Canada, and Synchro Alberta for annual demographic reporting, registration, determining age group, and to communicate with participants about synchronized swimming programs, events, and activities.
I understand that the Orca Synchronized Swimming Club, Synchro Alberta, and Synchro Canada have the right to take photographs, videotape, or digital recordings of club members and to use these in any and all media. I am aware that by giving this consent, I am permitting my/my child's name to be posted on the Orca Synchronized Swimming Club, Synchro Canada, and Synchro Alberta website and publications, which can be viewed by anyone who has access to these websites or publications. I understand that I may withdraw consent to the collection, use, or disclosure of my personal information at any time by contacting the Orca Synchronized Swimming Club, Synchro Alberta, or Synchro Canada.

3. Synchro Alberta Participant's Agreement or Waiver

Print the form, complete it, and bring it to the first practice. It must be signed before the swimmer can enter the pool.
*Note: Even if you have registered and paid the fees, until we receive a completed participant's agreement or waiver, we WILL NOT LET THE SWIMMER ENTER THE POOL. The participant's agreement or waiver is required for insurance and liability purposes.

Then, click on "Register" to e-mail your registration information to the Orca Synchro registrar.
You may receive alerts that an e-mail is being sent on your behalf. Please approve these alerts to send the message.
If an e-mail application opens and presents you with a mostly empty message to registrar@orcasynchro.org, this online registration form does not work with your browser. Please register by phone. You may still pay by credit card by using step 5.
5. Payment

Until the end of March, club members can guarantee a space for the fall by registering and paying a deposit of $60.
If your fee amount doesn't appear below, select the bottom option, "Pay some other amount."
Click on a Pay Now button to go to PayPal's secure website to pay by PayPal or credit card (Visa, MasterCard, Discover, or American Express). Orca Synchro will not receive your credit card information from PayPal. You do not need to create a PayPal account, nor do you need to sign in to your PayPal account if you have one. Instead, under "Don't have a PayPal account?" click Continue. If you have problems with PayPal, please contact them at the PayPal complaint web page or by phone at 1-402-935-2080 (not toll-free).
We request shipping information to collect your address via the secure PayPal website.
As well, for children, on the PayPal web page titled Review Your Payment click the plus sign beside "Swimmer's name and birthdate" to provide the child's name and birthdate to create an official receipt for income tax purposes. (If you forget, print the child's name and birthdate on the receipt and bring it to the treasurer to be signed.)
When you're done at PayPal, this portion of your registration is complete. Payment transaction details will be e-mailed to you by PayPal, and the transaction will appear on your credit card statement as PAYPAL *ORCASYNCHRO. Retain the receipt PayPal e-mails you for the federal children's fitness tax credit for children under 16.

New or returning Orca Synchro fall and winter synchro swimmers
For returning swimmers the fee for September 2008 to March 2009 is twice the amount paid in January 2008. The date when fractional fees apply has changed from January of the year to September, so the fees remain the same until September 2009.
